The “Nicosia
Squash Association” would like to inform all its members
about the new procedure and requirements regarding issuing a valid
Health Certificate (HC) which took effect from August 1st, 2009.
The new guidelines (points I – III) as they were send from
CSO follow. Please also give special
attention to the instructions given from NSA to all its members
(point IV).
I.
New ASYA’s “Special form”
As from August 1 only
the new ASYA’s “special form” will be valid.
This can be found attached along with the new requirements per
age group. The new guidelines and requirements as they were submitted
by the CSO follow.
II.
Necessary medical tests and clarifications
All necessary medical
tests are separated per age group and are as follows:
1. Under age
of 12:
ASYA’s “special
form” (filled in and signed by Doctor),
Cardio test ,
Blood type,
Copy of birth certificate
or Identification card or passports (for aliens),
2 recently taken photographs
(passport size).
2. Under age
of 35:
ASYA’s “special
form” (filled in and signed by Doctor),
Cardio test ,
Chest X-rays,
General blood test
and urine test,
Blood type,
Copy of birth certificate
or identification card or passports (for aliens),
2 recently taken photographs
(passport size).
3. Over 35:
ASYA’s “special form” (filled
in and signed by Doctor),
Fatigue test ,
Chest X-rays,
General blood test and urine test,
Blood type,
Copy of birth certificate or identification
card or passports (for aliens),2 recently taken photographs
(passport size).
In the case that any of the above are not attached
or not being correctly filled in, the application will be returned
to the sender with instructions for corrections. When the application
returns corrected, it WILL NOT get any priority.
Doctors eligible for examining and signings
ASYA’s “special form” are Cardiologists, Pathologists,
Pediatricians and Anesthesiologists.
Note that ASYA’s “special form”,
all analysis, cardio test and fatigue test can only be accepted
if issued in a prior 6 month period. Chest X-ray can be accepted
if made in a prior 12 month period. Please let us remind you that
the ASYA’s examination office for holding cardio tests for
all athletes is open every Wednesday from 15:00 – 19:00.
III. Time
needed for issuing a HC
The HC will be issued and delivered after twenty
(20) working days from the day of filling ones’ application
at CSO’s HC department (this is subject to the period of
time and load of work). The priority will be kept strictly and
NO interventions will be accepted.
IV. Guide-lines
to NSA’s members
For your participation in all Cyprus Squash
Association’s official events or where ever it’s
needed you should hold a valid HC.
Due to the new guidelines issued by CSO,
NSA incites its members to check their HC’s expiring dates
regularly and to proceed with the necessary medical tests for
issuing a new HC. Due to point III all medical tests should
be submitted to CSO’s HC department a month prior to the
expiration date.
HC should be submitted to CSO HC department
and collected by all members. NSA should not be involved.
Also a copy of the renewed HC should be submitted
to us with the number and expiring date clearly shown.
NSA cannot allow the participation of any
member at any event without a valid HC.
